Indications: Herpes Zoster: Famexavir (famciclovir) is indicated
for the treatment of acute herpes zoster (shingles), treatment or
suppression of recurrent genital herpes in immunocompetent
patients, and treatment of recurrent mucocutaneous herpes simplex
infections in HIV-infected patients.
Description
Famciclovir undergoes rapid biotransformation to the active
compound, penciclovir, which is phosphorylated by viral thymidine
kinase in HSV-1, HSV-2, and VZV-infected cells to a monophosphate
form; this is then converted to penciclovir triphosphate and
competes with deoxyguanosine triphosphate to inhibit HSV-2
polymerase (eg, herpes viral DNA synthesis/replication is
selectively inhibited)
Warnings/Precautions
Has not been established for use in initial episodes of genital
herpes, patients with ophthalmic or disseminated zoster, or in
immunocompromised patients with herpes zoster. Dosage adjustment
is required in patients with renal insufficiency. Tablets contain
lactose; do not use with galactose intolerance, severe lactase
deficiency, or glucose-galactose malabsorption syndromes. Safety
and efficacy have not been established in children <18 years of
age.
Contraindications
Hypersensitivity to famciclovir, penciclovir, or any component of
the formulation
Dosage and Administration
Adults : Herpes zoster: 250 mg three times daily for 7 days or 750
mg once daily for 7 days (500 mg three times daily for 10 days in
immunocompromised). Genital herpes: First episode: 250 mg three
times daily for 5 days; recurrent infection, 125 mg twice daily
for 5 days (500 mg twice daily for 7 days in all episodes in
immunocompromised). Suppression of recurrent genital herpes: 250
mg twice daily (500 mg twice daily in HIV patients) interrupted
every 6-12 months. Children: Not recommended
Interactions
Uricosurics: probenicid possibly reduces famciclovir excretion
thereby increasing plasma concentration
